Woodford County Health Department Immunization Clinic
Every Wednesday
7:30 - 11:30 a.m. and 1:00 - 3:30 p.m.
To receive immunizations at our clinic, you must provide a copy of your immunization records.
If a client is under 18 years of age, a parent/legal guardian must sign the paperwork. An adult must be present for the injection on clients under 18.
Not all disease can be prevented, but many can. The following vaccinations are offered at the WCHD:
- DTaP and TDaP
- MMR (measles, mumps, and rubella)
- MMRV (combination of MMR and Chickenpox)
- Meningitis
- Polio
- Chickenpox
- Hepatits B
- Haemophylus Influenza Type B
- Gardasil (HVP vaccine)
- Hepatitis A
- Rota-Teq
- Tetanus
- Prevnar
Vaccines for these diseases are also required for entry into Kindergarten, and Licensed Day Care or Preschool. The ages for vaccination are recommended. If a vaccine is not received at the recommended age it should be given as a “catch-up” vaccine at any next scheduled appointment. Vaccines may not be given earlier than the recommended age.
Childhood Immunization Schedule
Immunization Give at Hepatitis B (HepB) birth, 2mo, 1–4mo, 6-18mo
Diptheria/Tetanus/Pertussis
(DTaP, or Td)2mo, 4mo, 6mo,
15–18mo 4–6yrs,
booster dose every 10yrsH. Influenza type B (HIB) 2mo, 4mo, 6mo, 12-18mo Rota-Teq 2mo, 4mo, 6mo Polio (IPV) 2mo, 4mo, 6–18mo, 4–6yrs Measles Mumps Rubella (MMR) 12-15mo, 4–6yrs Varicella (Var) 12-18mo, 4-6yrs Hepatitis A 12mo, 18mo Varicella 4-6yrs TDaP 10-11yrs Meningitis 11-18yrs Prevnar 2mo, 4mo, 6mo, 12-15mo Gardasil (HPV vaccine) 9-18yrs The clinic schedule is every Wednesday.
Please note:
- Parent/Guardian must bring past immunization records with child.
- Parent/Guardian must accompany minor child in order to sign permit form.
